Consent to discuss your health records with someone else

At Sharnbrook surgery we want to reassure all our patients that your trust means everything to us and that your health records are handled with the utmost care, in line with strict UK GDPR Regulations and NHS Confidentiality Standards.

Therefore, we would like to inform you that if you are aged 13 or above and have previously given us permission to discuss your health records with a family member or carer, or another person, or you would like to give consent going forward, please be aware that we are no longer able to rely on verbal consent and will require the consent in writing by completing the form below.

This helps us:

  • Keep you records accurate and up to date
  • Make sure your wishes are clearly recorded
  • Continue share information only with the people you trust.

If, indicated as necessary we will request this written consent during your next interaction with us. However, feel free to complete and send to the surgery. We may also require photographic evidence if a member of the clinical team is unable to vouch for you. If the person consenting is authorised by a lasting power of attorney or has been appointed by the court of protection to make the decision on your behalf, we will require proof of this. 

In exceptional circumstances the clinician may proceed in your best interests without the written consent.
